HOSPITALIZATION

Hospitalization Procedure

  1. Please come to the Hospitalization counter at the 2nd floor. We will give you an orientation for your hospitalization.
  2. Please bring your Hospitalization permit, Medical examination ticket (ID card), Insurance card, Certificate of Eligibility for Ceiling-Amount Application and complete your hospitalization procedure by a specified time.
    • Please fill in the prescribed items on the hospitalization application form and submit it together with    your hospitalization permit to the hospitalization counter.
    • If you are over 75 years old, please bring your Health Insurance card for the elderly.
    • We will return your insurance card immediately after confirmation.
    • Please show us your insurance card at the hospital counter at the beginning of the month, since it is necessary to check it monthly even during hospitalization.
    • Please note that if you do not present the necessary documents for health care application such as   your insurance card, you may be required to pay the full hospitalization fee.
  3. Patients who are foreign nationality and do not have valid Japanese health insurance,20 yen (including tax) will be calculated for each medical treatment fee. Therefore, if you do not have an insurance card, please show us your passport or residence card.
  4. If you are subjected to Medical assistance, Rehabilitation medical care, Services and Support for Persons with Disabilities Act, Medical Treatment for Premature Babies, Specific medical care, Medical care for pediatric chronic specific illness, please let us know at the hospitalization counter.
  5. After completing the hospitalization procedure, please come to the nurse station in the ward where you will be hospitalized.

NOTICE

  1. If you are unable to complete the procedure due to Emergency hospitalization (outside of hours), please complete the hospitalization procedure at Emergency reception desk or at your earliest convenience the next day. (Next morning)
  2. To change or cancel the hospitalization date, please contact your doctor in charge, the ward nurse or the outpatient department as soon as possible.

What to bring for your Hospital stay

  1. Chopsticks, cups, forks and spoons
  2. Toiletries (toothbrush set, soap, shampoo, hair conditioner, etc.)
  3. Daily necessities such as tissue paper, Non-slip slippers, towels and bath towels (You can purchase some items at the convenience store)
  4. Underwear (including changing clothes)
  5. Sleepwear (If you use hospital gowns, we will charge you 74 yen per day.)

*Since the storage space is limited in the hospital, please keep your possessions to a minimum.

What to bring for your Hospital stay

  1. Change to hospital gowns (74 yen per day) or your own sleepwear.
  2. Please wear a certified wristband to prevent misidentification of patients. You need to confirm your name on the wristband with a nurse.
  3. Please confirm the name of the doctor in charge with a nurse.
  4. Other details of hospitalization will be explained by a nurse.

*We will give you a pamphlet of “Inpatient Information” for the details.

NOTICE

  1. Please refrain from parking your car at parking lot during the hospitalization because we need to keep spaces for outpatients.
  2. It is permissible to bring communication devices into hospital rooms. We allow mobile devices such as smartphones which is necessary for daily life, but you are not allowed to bring in devices that are unnecessary for medical treatment.
  3. You can use the TV or cooler box with a TV card. When using TV in your room, please ask your nurse or doctor. (Please use your earphones or headphones.) You can purchase a TV card at the vending machine.
  4. Please do not bring in large amounts of cash or valuables (jewels, accessories etc.) with you to prevent theft or losing your belongings.

    We do not take any responsibility for your damage and lost.

Your Hospital Stay

  1. Please follow the instructions from your nurse and doctor.
  2. We have following facilities inside the hospital. 
  3. No smoking on premises (including e-cigarettes and heated cigarettes).
  4. Be sure to write “floor, ward name, department name” in the address of the mail to the inpatients.

Meals

  1. The meals will be provided at the hospital according to each patient’s medical condition. Please refrain from bringing in your own food and drinks.
    • Breakfast: 8:00 AM
    • Lunch: 12:00 PM
    • Dinner: 6:00 PM
  2. If you have any food you don’t eat because of your religion or having allergies, please let your nurse know in advance.

Family Stay

Hospitalized patients are fully taken care of by hospital personnel, and their family members need not stay and sleep in the hospital room to look after them.
However, in some rare cases, a hospitalized patient’s family member may stay in the room if the patient’s doctor or nurse in charge permits. Doctor or nurse will explain the details.

Medical treatment during hospitalization

  1. In each department, the department chief, the ward nurse manager and others will cooperate with the doctor in charge to provide medical care.
  2. In order to train doctors and medical students, share information with medical teams, and provide better medical care, department manager regularly makes rounds.
  3. Depending on your medical condition, your room and ward may change. Please understand and cooperate with us.
  4. Make sure to receive sufficient explanation about the disease, examination, and treatment from your doctor or nurse.

Going out or Staying overnight

If you wish to go out or stay overnight, you need the permission of the doctor in charge. Make sure to submit a request for permission. We will issue a permit for going out and staying overnight. Depending on your medical condition, permission may not be granted. Please consult your doctor carefully.

Visitation

  1. Visiting hours: From 2:00 PM to 8:00 PM (Weekdays, Saturdays, Sundays and Holidays).
  2. Please be sure to stop by the nurse station first. Depending on the medical condition, we may have to refuse your visit.
  3. Please make your visits as short as possible in order to give each patient adequate time for rest.
  4. Please do not visit a large group of people.
  5. Young children have a weak immunity and easy to get sick so please do not bring young children.
  6. We limit visits during the epidemic of influenza. We may ask you to check your physical condition (fevers, coughs, runny nose and indirect pain) at the ward entrance before allowing you to enter the hospital room.
  7. We have a certain criterion to use cell phones or communication devices in the hospital. Please do not use in prohibited areas (where red stickers are attached).
